ICA Medical Examination Report
A chest X-ray for active tuberculosis and an HIV test, certified by the examining doctor. Every field must be completed, with no extra remarks, as ICA rejects reports that don't meet its requirements.
ICA & MOM medicals · Singapore
The chest X-ray and HIV test for ICA's medical report, or the form in your MOM letter, done at Wheelock Place. Bring the letter first: it tells us what you need.
Two checks, one report: a chest X-ray and an HIV test.
ICA's Medical Examination Report, used for Permanent Resident and Long-Term Visit Pass applications when your letter asks for it, has two checks: a chest X-ray for active tuberculosis and an HIV test. The examining doctor records both results, then signs and stamps the form. ICA accepts it only if it is submitted within three months of issue.
Dependant's Pass holders follow MOM instead: the in-principle approval letter states whether a medical examination form or a medical declaration is needed before the pass is issued.
Pregnant? Tell us before the X-ray. The form allows an exemption during pregnancy, but ICA asks for a certified X-ray once you are no longer pregnant.
Which form applies
Start with the letter that came with your approval. ICA's pathways for PR and LTVP use its Medical Examination Report, while MOM's Dependant's Pass uses the form or declaration named in the IPA letter. A renewal, a pregnancy or a recent medical report can change what is needed.

The form in your IPA letter
MOM's letter states whether you need a medical examination form or a medical declaration. Bring the letter and the form; the completed document is uploaded when the pass is issued.

Renewing, or examined recently?
Renewal applicants must be examined in Singapore. If you submitted a medical report to ICA or MOM in the past two years, ICA's form has a declaration you may be able to sign instead of repeating the examination.

X-ray and blood test in one visit
Our X-ray room and blood taking are at the clinic, so both checks on ICA's form are done during your visit. The doctor signs the report once the results are back.
Who it's for
Only applicants whose letter asks for one. The letter, not the pass name, tells you which form to bring.
Pregnant, or might be? Tell us before any X-ray.

Questions
The letter tells you which form. The form tells us which tests.
Bring your original passport, the unaltered form and your approval letter, and leave the doctor's sections blank.
No. The issuing authority, the pass type and your approval or IPA letter decide whether you need a medical examination, a declaration, a chest X-ray, an HIV test or another document. Start with the letter: it names the form to bring.
ICA's Medical Examination Report covers two checks: a chest X-ray for active tuberculosis and an HIV test. The examining doctor records both results, then signs and stamps the form, and may not add extra remarks. In Singapore, the HIV test can be a rapid test or an ELISA test.
ICA accepts the report only if it is submitted within three months of the date it is issued. Plan your appointment so you can submit in time, and check any other deadline in your own letter.
For ICA's report, first-time applicants may be examined in their home country or place of residence, but renewal applicants must be examined in Singapore. MOM passes may follow different rules, so check your letter before booking overseas.
Tell us before any X-ray. ICA's form allows the X-ray to be exempted during pregnancy, but once you are no longer pregnant, ICA asks for a chest X-ray report certified by a Singapore-registered GP. Bring any pregnancy documents to your appointment.
Possibly not. ICA's form includes a declaration for applicants who submitted a medical report with the same information to ICA or MOM not more than two years ago, when they were granted a pass. If that applies, you may sign the declaration instead of repeating the examination. Bring the earlier pass details if you're unsure.
No. A screening package is designed around your health, not around ICA's or MOM's form. Bring the authority's form, so the doctor completes exactly the fields and tests it asks for. If you'd like a broader health check as well, ask about our screening packages separately.
